The formula

d = √((x₂ − x₁)² + (y₂ − y₁)²)

The horizontal and vertical gaps form the legs of a right triangle, and the distance is the hypotenuse. That is the whole derivation, and it extends directly to three dimensions by adding a Δz² term.
