All of these describe the same person. Which one you use is entirely regional: centimetres almost everywhere, feet and inches in the US and UK, and decimal feet mainly in engineering.

Why medical forms want centimetres
Formulas such as BMI, body surface area and lung-function predictions are defined in metric units, so a height in feet and inches has to be converted before it can be used. Doing that conversion once, accurately, avoids the rounding errors that creep in when you convert at each step.
